Job overview

An exciting opportunity has arisen for a Band 5 Occupational Therapist post in the Cornwall, Central Integrated Area Team. The post holder will be required to work in the Community and Home First Teams providing assessment, reablement and rehabilitation to adults with health problems. This includes creating and reviewing care plans, reablement plans to facilitate independence and self-management and rehabilitation plans for support workers to implement. The teams support the Ageing Well Programme with 2 Hour Urgent Care Responses, 48 Hour Reablement Responses and Anticipatory Care Responses. The teams have key roles in supporting prevention of admission to Bedded Care and Discharge to Assess pathways. The teams participate in the assessment and supporting the discharge plans for people in Bedded Care settings.

We are seeking innovative pre and post registration Clinicians who will relish this creative approach and career development within the wonderful county of Cornwall.

Structured supervision and opportunities to participate in the established in – service training programmes and support for continued professional development will be provided

This is a full time post working 37.5 hours per week.


Main duties of the job

Responsible for the assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.

Essential criteria
  • Degree or Diploma in Occupational Therapy
  • HCPC registered
  • Professional Qualifications taken in English Language or appropriate qualification from the International Language Testing System (IELTS)

Experience

  • Experience of working within a multidisciplinary team.

Skills & Aptitude

Essential criteria
  • Well-developed communication skills – empathy, negotiation, motivation and presenting of complex facts in an understandable way, re-assurance, and support particularly to anxious people
  • IT skills to include use of Word, Excel, email, internet, recording systems
  • Understanding of conditions that can result in long term disability
  • Understanding of the roles of colleagues in health and social care
