INTERVIEW DATE-MONDAY 22ND MAY 2023 West Cornwall Adult Community Services are looking for 2 x Band 6 Long Term Conditions Nurses to work with the Penwith Community Matron team and alongside our integrated team of nurses, therapists, support workers and administrators. The Penwith area comprises: Hayle, Marazion, St Ives, Penzance and surrounding villages.

We are looking for dynamic, caring and motivated nurses with a proven track record of Community Nursing and who also have Long Term Conditions experience either holding their LTC certificate or working towards attaining the qualification.

This role will comprise 2 part time posts - 30 hrs per week and 22.5 hours per week, details to be discussed at interview.

The ability to travel independently across the West PCN area to fulfil the requirements of the post is essential, this includes a requirement for flexibility as the service aims to be available 7 days a week 8 am to 8 pm. As with all healthcare services we will be moving toward a 24/7 service provision model.
The post holder will be the keyworker for patients within the virtual ward or integrated team who require additional support and management of their Long-Term Conditions.

Main duties of the job

As an autonomous practitioner the post holder will be responsible for the assessment and treatment of patients with complex health and social care needs. Ensuring provision of a holistic nursing assessment and high quality, safe, evidence-based care to patients and carers with the aim of maintaining patients in their own homes. This includes physical examination and detailed history taking, diagnosis of deterioration in pre-existing conditions and treatment planning.

The post holder will work collaboratively with the Community Matrons, Community Nurses, Practice Nurses and Discharge and Eldercare Co-ordinators managing patients with chronic disease and frailty through autonomous practice including taking managed clinical risks with the ability to discuss or escalate to a Community Matron. E.g., where the patient is kept at home or in a care environment, rather than hospital admittance as would previously been the practice.

The post holder will work in this adaptable and evolving role, focusing on safe, effective admission avoidance and supporting timely discharge from acute hospitals, with a whole systems approach. The post holder will co-ordinate care and strive to reduce the risk of acute re-admissions by identifying risks, implementing management plans and collaborating with the whole community resource to facilitate ongoing care management.
